Notes to keep in mind
- Not all policies run calendar year (January - December).
- Most plans cover a monetary amount while some may indicate how many session per service type.
- Some plans may require a deductible to be paid by the patient to the clinic at the start of each policy year unless stated otherwise.
- Some plans may require a prescription from a medical doctor before receiving treatment to pay for services rendered.
