Please contact Manitoba Public Insurance (MPI) at 204-985-7200. You may also call this number if you're not sure if you qualify for coverage with MPI.
Make sure you have enough time to wait on hold in case of call-in backlogs due to recent spikes in motor vehicle accident (MVA), etc.
Injury claims with MPI must be approved before scheduling an appointment with Academy Athletic Therapy Inc.
Information Required for Direct Billing:
- Patient first name
- Patient last name
- Date of birth
- Injury claim number
- Date of loss ( car accident)
- MPI case manager ( if assigned)
- MPI case manager phone number
Patients will also receive forms specific to the injured sites for medical documentation.
